Office Hours – Elemental Health Care
Our office hours are Monday from 9 a.m. to 2 p.m. To schedule an appointment, please call 480-659-1390. If you are a new patient, please fill out the New Patient Information form and bring it with you to your appointment. Once you become a patient with Elemental Health Care, you may request an appointment by clicking on the Scheduling button on this website. First time users will need to create a login profile.
